I am neither a psychologist nor psychiatrist but your phrase "it's only inside his own head" triggered a thought. So I checked and Trump exhibits all of the nine signs of a Narcissistic Personality Disorder listed in DSM 5 (only five are necessary for a clinical diagnosis). NPD is notoriously difficult to treat. He would likely rather see the United States devolve into a bloody civil war rather than ever concede defeat. Sadly, he is likely be with us and creating chaos until he is either locked away in a secure facility of some kind or the first spade of dirt hits his coffin.
