Update Joe others/ problem solved, 2 hours with senior supervisor Apple:
The bluetooth icon went away after last ios update, so it was mileading all time:
The fix: was unchecking preference in the iphone to "allow call on other devices".
The dang thing was making the speakerphone icon go to that menu at bottom of iphone with imac, latptop, instead of just speakerphone working. Nasty one.
