credit card breach

For the fourth time in two years, Discover has issued a new card to me because of a data breach that, according to Discover, did not involve Discover systems. While I understand the necessity of doing this, it's a royal PITA. There are a few websites that store my credit card information, namely PayPal, NY Times, E-Z Pass, Consumer Reports, App Store, and Amazon. I prefer to not have this info stored on other sites, and do it for these because of the convenience.
My question: How do these breaches occur, and is there anything that I can do to prevent them? BTW, I have not noticed unauthorized charges (and I check regularly).

My question: How do these breaches occur, and is there anything that I can do to prevent them? Like you, I use PayPal, which means I enter my credit card information at fewer sites but I still wonder how "safe" that really is. Let's face it, if the hackers can crack places like the military and the NSA how "safe" is any other site? In Canada they cracked the Tax Department which I considered to be about as safe as you could get. And, what about purchases not on-line? I recall going to an appliance parts store where I had been a couple of years earlier and, when I was presenting my credit card, the salesperson said "Not necessary. We still have the number from the last time you were here". So, maybe having to change your card once in a while, although a royal PITA, is not such a bad thing.
