customer just brought in his macbook, he's managed somehow to get Word to make ALL new documents with a page layout of 4.5" wide. (instead of 8.5")

The margin bubble at the top goes to only 4.5". If you double click that it opens margin setup and confirms the margins are 0.5 and 1.0" around the borders. Clicking page setup from there takes you to the page setup which confirms the page is 8.5 x 11.

This guy is well known for finding the most obscure settings in software and changing them to cause bizarre unexpected behaviors, so I'm pretty sure he's done it again, but this time I can't figure out where he found the setting. I'd normally delete the "dot normal" file but can't find it in this version of word. Clicking the "defaults..." to reset it doesn't help either.

I don't know if this is the same "normal" that you seek but in my Office 2008 installation, there is a Normal.dotm file located in ~/Library/Application Support/Microsoft/Office/User Templates. Have you tried trashing (or moving) ~/Library/Preferences/com.microsoft.Word.plist?
